Summer Camp Enrollment Forms

We’re excited to have your child join Albany Preschool’s Summer Camp program.

The forms linked below are required by childcare licensing before a child can attend camp. They can be emailed to info@albanypreschool.org, dropped off at the preschool (there is a mail slot on the door for after-hours drop off), or mailed to Albany Preschool, 850 Masonic Ave, Albany 94706.

Please note that child care licensing requires that we have legal signatures where a signature is required, and that all questions are filled out. Please be sure to write “none” on the Consent for Medical Treatment form if your child has no known allergies. 

1. Summer Camp Parent Agreement - The Administrative Coordinator will email your agreement which will list your child’s camp sessions.

10. Physician’s Report - The Administrative Coordinator will let you know if this form is required for your child. It must be signed by your child’s pediatrician.

11. Immunization records - If the Physician’s Report is not required, we need a copy of your child’s immunization records (for example, photocopy/scan of immunization card, screenshot of online records, etc.) A list of required immunizations is here, and you may be able to look up your child's records on California’s Digital Vaccine Records site here.

12. Physician’s Report - The Administrative Coordinator will send you more information if this form is required for your child. It must be signed by your child’s pediatrician.

13. Tuition:

    • Our bookkeeper will email you your tuition invoice with the due dates specified in the Parent Agreement

    • Tuition invoices will have a link to pay online via a bank account, or a check can be dropped into the tuition mailbox located on the windows above the cubbies.
