Summer Camp Enrollment Forms

We’re excited to have your child join Albany Preschool’s Summer Camp program.

The forms linked below are required by childcare licensing before a child can attend camp. They can be emailed to info@albanypreschool.orgor dropped off at the preschool (there is a mail slot on the door for after-hours drop off).

Please note that childcare licensing requires that we have legal signatures where a signature is required, and that all questions are filled out. Please be sure to write “none” on the Consent for Medical Treatment form if your child has no known allergies. 

  1. Summer Camp Parent Agreement - The Administrative Coordinator will email your agreement which will list your child’s camp sessions.

  2. Identification & Emergency Information for Childcare Centers

  3. Preadmission Health History

  4. Consent for Emergency Medical Treatment- be sure to write “none” on the form if you child has no known allergies

  9. Copy of Immunization records - For example, photocopy/scan of immunization card, screenshot of online records, etc. A list of required immunizations is here, and you may be able to look up your child's records on California’s Digital Vaccine Records site here.

  10. Sunscreen Permission (Full Day/Reduced Full Day families only)

  11. Physician’s Report - The Administrative Coordinator will send you more information if this form is required for your child. It must be signed by your child’s pediatrician.

  12. Tuition:

    • Our bookkeeper will email you your tuition invoices on the following schedule:

      • June tuition is emailed the last week of April and is due May 1

      • July tuition is emailed the last week of May and is due June 1st.

    • Tuition invoices will have a link to pay online via a bank account, or a check can be dropped into the tuition mailbox located on the windows above the cubbies.
