School Year Enrollment Forms

Welcome to Albany Preschool!

The forms linked below are required by childcare licensing before a child can attend. They can be emailed to info@albanypreschool.orgor dropped off at the preschool (there is a mail slot on the door for after-hours drop off) or mailed. Our mailing address is Albany Preschool, 850 Masonic Ave, Albany 94706.

Please note that childcare licensing requires that we have legal signatures where a signature is required, and that all questions are filled out. Please be sure to write “none” on the Consent for Medical Treatment form if your child has no known allergies.

  10. Physician’s Report - This form must be signed by a parent/guardian as well as by your child’s pediatrician. Some providers may have their own version of the form which is fine. Please let the Administrative Coordinator if there is a delay in getting this form from your child’s doctor and turn in the rest of the forms, including the immunization records.

  11. Immunization records - Immunizations can be entered on the Physician’s Report or turned in separately (photocopy/scan of immunization card, screenshot of online records, etc.) A list of required immunizations is here, and you may be able to look up your child's records on California’s Digital Vaccine Records site here.

  12. Tuition: Our bookkeeper will email you your tuition invoice which will have a link to pay online via a bank account, or a check can be dropped into the tuition mailbox located on the windows above the cubbies.

MORING PROGRAM FAMILIES ONLY:

13. If you family has signed up for Classroom Participation, state law requires that we have the following documents on file for the adults that will be participating for your family:

  • Health Statement form

  • Negative TB test administered within the last 12 months (skin, blood or x-ray)

  • Proof of measles immunization or immunity

  • Proof of pertussis immunization

  • Proof of a flu shot administered after September 1st of this year will be due by December 2nd. Don’t worry, we’ll send a reminder. Alternatively, this is the one requirement that allows for a personal exemption in which case we must get a signed and dated note that simply says, “I have declined the influenza immunization.” You can print out and use this note here.
